It is important to understand your financial responsibility. Our office no longer accepts insurance. All appointments are self pay.

Regardless of your insurance status, you are financially responsible for treatment provided to you and/or your legal dependent by this office.

Health Savings Account (HSA) and Health Reimbursement Account (HRA)

An HSA or HRA account may allow you to pay for our self pay services. These accounts usually work like a preloaded debit card and simply need a receipt for services submitted to maintain reimbursement. Check with your individual accounts for restrictions and eligibility.
